In a flashback to fifteen years ago, it is revealed that John Garrett (a S.H.I.E.L.D. agent who is a member of HYDRA) pulled Grant Ward out of a Massachusetts juvenile jail after Ward broke out of a military training camp, drove across the country, and then set his house on fire, with his older brother still inside. Garrett gives Ward the choice to either stay in juvi or go become an agent. Ward accepts the offer to become Garrett's protégé. S.H.I.E.L.D. officers overpower the local guards, and the elder and now recruited agents leave.

Today, Phil Coulson and his team, Agents May, Triplett, Skye, Fitz, and Simmons, are in a motel outside Los Angeles. They realize that in order to track the location of Coulson's old plane (now in Garrett's possession), The Bus, they will need to plug a USB drive with one of Skye's programs into any computer belonging to Cybertek. Cybertek is one of Ian Quinn's many companies, and their database and HYDRA's are closely linked, so Skye's virus will have full access. Triplett opens a briefcase containing some of his grandfather's gear from the Howling Commandoes. The team will use this to break into Cybertek.

Again flashing back fifteen years, Garrett drops off Buddy (his dog) and Ward in the New England woods. The elder S.H.I.E.L.D. agent tells Ward that he had better learn how to be a man and care for yourself, and leaves.

Meanwhile in the present, Coulson's team is infiltrating Cybertek. With headphones that relay Fitz's and Simmons' voices to them, Coulson and May go for a job interview at Quinn's company, posing as young scientists. Presenting their "icer" bullet for the "Night-Night Gun" (to which the interviewers are unimpressed) Coulson and May gain access to the facility. Even though they are declined, they still manage to look around the offices - the two S.H.I.E.L.D. agents kill the person guarding the elevators and then take out the guard outside Cybertek's file storage room. Much to their surprise, however, the files are in paper, not digital form, and they begin to rifle through them. Opening the cabinet for Project DEATHLOK, they realize that Garrett, in 1990, became the first Deathlok, not Mike Peterson as first anticipated. They heave the filing cabinet out the window and zip-line down to Triplett, making their escape.

At Ernesto's barbershop in Havana, Cuba (which is one of HYDRA's secret bases), Garrett tells everyone that they are packing up and leaving for new facilities in the United States. Garrett collapses, though, and we learn that his body and mechanics that were implanted at the beginning of Project DEATHLOK are failing - he is going to die. Ward reboots Garrett's implants and he is back on his feet, temporarily.

In another flashback, Garrett finds Ward in the woods. The junior agent has stolen from other cabins in the park and learned to hunt. Garrett pulls out a revolver, shoots up a few pans, and promises Ward that the next stage in his training will begin.

In the present, Coulson's team has figured out where Garrett's hideout is somewhere in Havana, and they use their current plane (which used to be Garrett's) to fly to Cuba. Fitz and Simmons go to the airport to locate Coulson's plane, while the rest of the team searches through town. They find Ernesto's barbershop, but it is deserted - except for a few CENTIPEDE soldiers.

It has now been a couple of years in the flashback timeline, and Ward has learned to become a deadly agent. His mentor tells him that he has had a certain dislike for S.H.I.E.L.D. ever since he was brutally injured during the Bosnian War, in 1990. Headquarters would not send a MedEvac chopper, and Garrett had to duct-tape himself together. He informs Ward that he is really working for an organization within S.H.I.E.L.D. - HYDRA. Garrett tells him that it's time to go - but only after Ward kills the dog Buddy. Initially, Ward fires a shot into the air, and Buddy runs away, but later we see someone looking at the dog through a sniper-scope, leaving it up to the viewer to decide what happened to Buddy.

At a Havana airport, Ward finds Fitz and Simmons spying on HYDRA loading the plane. They come on board, and Fitz uses a mini EMP from Triplett's gadgets to fry Garrett's DEATHLOK electronics. Garrett, enraged, tells Ward to kill them, and then gets Raina to open up his chest-plate. Fitz and Simmons initially manage to break free from their captors, but eventually Ward comes and they are forced to lock themselves inside a storage pod. Ward tries to convince them to open it, but they refuse. Ward cannot bring himself to personally kill his former teammates, so he starts an ejection sequence to drop the storage pod from the plane. Fitz and Simmons urgently plea for Ward to not do it. Fitz screams that he believes Ward still cares about them. Ward quietly states that he does indeed still care about him and Simmons, and says that it's a "weakness". The storage pod, with Fitz and Simmons in it, dumps off the Bus, and into the ocean below. Back with Garrett, Raina learns that he will die without every drop of the GH-325 serum that she has concocted using samples from the "Guest House" facility and Skye's blood data. She injects Garrett. He goes into a seizure but survives.

At the barber shop, meanwhile, Coulson's team is cornered by a team of HYDRA agents, three who are Centipede soldiers and one of which is carrying the Berserker staff.

In the final scene, Ian Quinn is in Washington talking to two leaders of the US Military, offering to sell them a thousand super soldiers just like Deathlok, while mentioning that the South American drug lord's assassination was part of the Deathlok 'road test'.
